General Information
DISD policy states a student must be fever free without medication for 24 hours before returning to school. Please do not send your student to school if they are experiencing persistent signs of infection (diarrhea, vomiting, discharge from eye, etc.). The CRC office will accept a handwritten note outlining dates and the reason for absences.

- Medication in Clinic Request -- if your student needs medication kept in the clinic during the school day please have your doctor complete this form along with your signature at the bottom Medication Form
- Special Diet Request -- if your student has dietary allergies or an intolerance, please have your doctor complete this form along with your signature at the bottom Special Dietary Needs
- Immunization Requirements -- please refer to this list to ensure your student is up to date on vaccinations Immunizations
